New York State Prescription Drug Program

New York's Senior Prescription Drug Program for seniors 65 or older went into effect on January 1, 2001.  To apply for this program you can either phone 1-800-332-3742 or fax 1-518-452-3576.  The program is open only to New York State residents.

You are eligible if your last year's earnings are no more than $35,000 for a single individual or up to $50,000 for a couple.  Depending on your earnings, you are assigned a deductible.  After this deductible is met, your drug costs are as follows:

Store| Price	You Pay (after deductible)
up to $15.00	$3.00
from $15.01 to $35.00	$7.00
from 35.01 to $55.00	$15.00
Over $55.00	$20.00
